Source document download URLs for a plan

/plans/:id/documents

Returns the plan's source documents (plan description, map, illustration, decisions) each with a fresh, time-limited download URL. URLs are minted on demand and expire ~1h — fetch the PDF promptly, don't cache the URL. The documents themselves are not stored by Placepoint.
